Case Management - Developmental Disabilities

Curative's Developmental Disabilities Case Management Program - through the Milwaukee County Disability Services Division - provides case management and support services to persons between the ages of 18 and 59 who have been diagnosed with a developmental disability. Services foster independence in the home and community.

Through the Medicaid Waiver Program, Curative's Developmental Disabilities Case Management team assists individuals and their families in locating, managing, coordinating and monitoring all proposed services and informal community supports. Services under the Waiver Program include:

- Development of an Individual Service Plan

- Completion of functional screening

- Arrangement for provision of services

- Service Coordination

- Assessment

- Crisis intervention

The Case Management Team includes the program member, a Curative Case Manager assigned to provide assistance and a Curative Case Management Manager.

Personal Planning Service

Curative offers a Personal Planning Service to assist persons with disabilities who are on the Milwaukee County Adult Services Waiting List. Services are provided at no cost to individuals with disabilities.

Curative's Personal Planner works with an individual and family members to identify areas of interest, needs and a future vision, and to identify supports necessary to accomplish goals. Based on needs and goals identified, the Personal Planner researches and gathers information on available community resources and recommends helpful strategies to accomplish goals.
