Children's Long-Term Support (CLTS)

Milwaukee County Disabilities Service Division contracts with Curative Care Network to provide Service Coordination for children and families currently participating in the Children’s Long-Term Support (CLTS) Waiver Program and the Children’s Community Options Program (CCOP).

The Curative team provides service and resource coordination for children with severe physical, developmental or emotional disabilities who meet the criteria of the CLTS Program. 

Children served must be residents of Milwaukee County.

To apply for the Children’s Long-Term Support (CLTS) program, please contact your County Agency. The Milwaukee County Disability Services Division (DSD) intake line is available Monday - Friday from 8:00 a.m. - 4:30 p.m. by calling 414-289-6799.

A partnership dedicated to service and support

Curative staff work with children and families plus other organizations to maximize access to vital community resources. All services are based on each child's assessed needs and goals, which are developed with input from the family and/or caregiver.

Persons served 

  • Curative Service Coordinators serve children up to age 18 who meet the criteria, including but not limited to: an appropriate disability determination, financial requirements, and level of care through assessment and functional screen.

Core Team Members

  • A professional Service Coordinator is assigned to each child and family to help assess their individual needs and arrange for appropriate services to meet those needs.

Curative Care Network is committed to providing high quality, person-centered care and services. Our program outcomes are reviewed monthly to develop focused actions for continuous performance improvement.

Achieving Outcomes

Curative’s clients and their families work with our staff to assess what is important to the family. The team also establishes individual goals and staff-supported achievement plans. Our customer experience feedback is used to develop actions for continuous program improvement. Hours and location

Visits with Curative Service Coordinators are typically conducted Monday through Friday from 7:30 a.m. to 4:30 p.m. Evening appointments may be available upon request. Based on the needs of the clients and families, services are primarily provided in the community.
